Supporting patients to achieve their wish to die at home: System supports and symptom control in palliative care

Join our health care experts to learn how to prepare your patients and their families for palliative care, understand system supports and symptom control, what services and resources are available, and the best referral pathways.

This free accredited session will be held on Tuesday 18 June 2024 from 6.15pm at the Global Streaming Centre of The Hub, Bentley Technology Park. This session will provide you with 1.5 CPD hours and includes a buffet dinner and refreshments.

Silverchain Medical Director, Palliative Care WA, Dr Fiona Findlay, will lead the presentation and be supported by her experienced palliative team. They will share their knowledge and support you to work through different case studies during the session.

  1. Distinguishing between palliative care options available to support patients with their preference
  2. Understand the referral pathways for home based palliative care and how to optimise outcomes for patients choosing to die at home
  3. Develop a better understanding of community based services and resources available for patients and families.
